im running a t3/t4 TO4E and i need to decide on either a 35 mm or 38mm wastegate.
does bigger "mm" mean better performance?

yes...
although the 35 may be cheaper.. the tial flanges are all 38mm (well the ones for the 35 or 38mm wastegates)

forgive the idiotness, but what is the difference other than the size?? I've always wanted to know that?
