To install a JBP cam on a 2.2L does the engine have to be pulled from the car? If not how would it be done?
Also with a new cam are new lifters manditory?

Simple question simple answer YES
it doesn't necessarily have to be pulled out of the car, but in the end run you're better off doing it that way. I haven't done this first hand but the general consensus so far as i've researched is that most people who have done it in the car say they'd rather have pulled the engine after all was said and done. And yes, you should replace the lifters as well. No point in risking scuffing up that nice new cam just to save a few bucks.

well you have to take the head off.... head covers the lifters and holders.
dont tell me you are working on a car WITHOUT a manual?
