I've been having problems with my O2 sensor ever sence I installed my header, but noticed that my cooling fan runs all the time as soon as I start the car up, totally cold, it comes on and never shuts off. What have I done now?
unplug the battery for 20 minutes, the pcm will reset. and then it should work just fine.

If disconnecting the battery doesn't work, you might need a new o2 sensor on the header or the one behind the cat converter is bad and needs to be replaced. This just happened to me recently.
