A bad O2 sensor. How do you know it's running rich?

That mean it's normal. If you ever see a car with a clean muffler, don't buy it.

Also, the narrowband is not good enough to say your car is running rich. The car run rich from factory as a protection.

Our cars only run rich at WOT or when cold. Otherwise, pretty close to 14.7/1. My muffler isn't black on the inside. That's definitely abnormal. Maybe one of your injectors is injecting more fuel than another? If one of your cylinders is running lean and dumping O2 down the manifold, the other three cylinders will run rich. Try some injector cleaner or seafoam.
